What documents are needed for a residence card through MOS

After the move to electronic submission through MOS, many foreigners think that filling in the online form is enough. In practice, the document package remains essential. In general, it is worth preparing a passport, documents confirming the basis of stay, proof of address, income, health insurance and payments where required.

If the case is handled by a representative, a power of attorney and proof of the 17 zł fee may also be needed. In family cases, marriage certificates, birth certificates, documents of the spouse or parent, translations and proof of the family situation may be important.
